Please give your event a name if it does not already have one and include it on this form.
• Contact person is person requesting this event to be scheduled.
• Please include address, phone number and e-mail address for this person. This information is required and will be kept confidential.
• Also include the organization you are representing. Within the church this may be the team, guild, or committee you are representing on this form. Outside groups may include Boy/Girl Scouts, Self-Defense or other athletic group, wedding, etc.
• Type of event may be meeting, meal or food involved, fundraiser, etc.
• We need the MAXIMUM number expected to attend.
• Give us the room you prefer as well as any additional rooms or resource needs you may have. These might include tables & chairs, VCR, etc.
• We must have both the starting and ending times for your event and any additional time needed for set-up and clean-up. Please give the maximum amount of time you expect to need. We reserve the right to schedule any times beyond what we confirm for your schedule.
